Performance Anxiety: Breaking the Feedback Loop

Performance anxiety is the most self-reinforcing problem in men's sexual health: the fear of failure directly produces the failure, which increases the fear. The good news is that a loop with a physiological mechanism has physiological exits.
The adrenaline problem
Erections require a parasympathetic state — the 'rest and digest' side of the nervous system. Anxiety activates the sympathetic side, dumping adrenaline and noradrenaline, which constrict smooth muscle. That is the exact opposite of what nitric oxide is trying to do. You are, biochemically, fighting yourself.
How to spot it
Strong clues that anxiety is the primary driver: erections are fine alone or on waking, the problem started suddenly rather than gradually, it's worse with new partners, and it's worse when the stakes feel high.
Things that measurably help
None of these are quick fixes, but they have real evidence behind them.
- Sensate focus exercises — structured touch with intercourse deliberately off the table for a few weeks
- CBT or sex therapy, individually or with a partner
- Naming it out loud to your partner, which removes the surveillance feeling
- Reducing alcohol, which people use as an anxiolytic but which worsens erections
- Mindfulness and slow breathing before and during sex to shift nervous system state
Where medication fits
For some people, a short course of prescribed PDE5 medication acts as a circuit breaker: a few reliable experiences reduce anticipatory anxiety enough that confidence returns. That is a legitimate clinical strategy, but it should be a provider's call, not a self-prescribed one, and it works best alongside the behavioral work rather than instead of it.
