Testosterone and ED: Where It Helps and Where It Doesn't
September 4, 2026 · 6 min read
Testosterone clinics market heavily on ED, but the clinical picture is narrower than the advertising. Understanding the difference between libido and erectile mechanics saves a lot of money and a lot of unnecessary injections.
What low testosterone actually causes
Genuine deficiency (hypogonadism) typically reduces sexual desire, energy, mood, muscle mass and bone density. Erection quality can suffer, but usually as a downstream effect of lost desire rather than a mechanical failure. Men with normal levels and ED rarely improve on testosterone therapy.
How to test properly
Total testosterone should be drawn in the morning, ideally twice on separate days, because levels swing significantly through the day. A provider may also check free testosterone, LH, FSH, prolactin and thyroid function to find out why a level is low rather than just that it is.
What lowers it besides age
Age is the least interesting cause on this list.
- Obesity, especially visceral fat
- Obstructive sleep apnea
- Chronic opioid use
- Anabolic steroid use, past or present
- Chronic illness, severe stress, and long-term undernutrition
The risks nobody advertises
Testosterone therapy suppresses natural production and sperm count — a real problem if you may want children. It can thicken the blood, worsen sleep apnea, and requires ongoing monitoring. It's a legitimate treatment for diagnosed deficiency and a poor first move for ED with normal labs.
The short version
Test if you have desire, energy and mood symptoms. Don't assume testosterone is the answer to an erection problem with a normal panel.
